Milan got off to a flying start at San Siro when Santiago Giménez – playing against the club he left in January – headed in from a corner after only 36 seconds.
The Rossoneri continued to press for a second throughout the first half, with João Félix sending his shot just wide and Theo Hernández hitting the post with his attempt.
Into the second half and with the tie delicately poised at 1-1 on aggregate, Hernández was sent off for his second yellow card, this time for simulation.
Feyenoord then struck, as Hugo Bueno floated in a superb cross towards the back post, which was met with a strong header from Julián Carranza to put the Dutch side ahead overall in the tie.
